  As part of the rental process, you will be required to leave your valid student ID card at the SSA office. All information will be kept confidential by SSA Student Services, and will only be used to prevent loss or theft of rentals.

  By submitting the form, you acknowledge and accept your responsibilities as a renter to return the borrowed item on time and in good condition. Failure to return the item within 48 hours will result in a charge to your student account associated with your UM email. Please fill out all the information to receive the rental.
